Dear Student:
Thank you for your consideration in applying for a
scholarship award. Please make sure that you meet the eligibility
requirements before you proceed. By applying for an award, you are
authorizing the Scholarship Fund Board to verify your GPA and any
other eligibility requirements as necessary, and to contact your
Program Chair as necessary.
In the interests of fairness and neutrality, your
identity and the program you are attending will not be revealed
during the selection process. The Scholarship Fund Board is
comprised of Past-Presidents of NJHIMA, including Susan DeSantis,
Chair. Only the Chair will know your identity and program, and she
will not participate in the final selection.
If you have any questions or concerns, please direct
them only to Susan DeSantis, email SDESANTIS.IHMI@COMCAST.NET.
Please do not discuss, show, or send your completed application to
anyone on the Scholarship Fund Board other than Susan DeSantis.
APPLICATION DEADLINE IS APRIL 16, 2010.
